Services are provided following a veterinary referral form under The Veterinary Surgery (Exemptions) Order 2015 (SI2015/772) in the Veterinary Surgeons Act (1966).
A veterinary referral form certifies your vet has given permission for physiotherapy treatment to be provided for the specified patient. Prior to gaining veterinary referral, you will be asked to contact your vet to give your consent for pet details to be passed on to me.
Upon first appointment you will be required to complete an owner consent form, certifying that you give permission for me to provide physiotherapy for your animal.
The cost of each session shall be paid in full via cash or bank transfer at the time of appointment. If payment is not received within five days after being reminded that payment is due, services will be suspended until payments are made.
